Divarasib raises median overall survival to 17.7 months

The global Phase 3 Krascendo 1 trial enrolled 338 patients at 151 centers in 26 countries with advanced or metastatic KRAS G12C-mutant non-small cell lung cancer that had progressed after prior treatment, directly comparing divarasib with sotorasib or adagrasib. Median overall survival was 17.7 versus 12 months, a 44% lower risk of death; median progression-free survival was 9.8 versus 5.7 months, a 42% lower risk of progression. Objective response was 58.4% versus 32.3%, while severe treatment-related side effects occurred in 25% versus 35%. Results were published in The New England Journal of Medicine and presented at ESMO 2026. Limits include the open-label design, a smaller adagrasib subgroup, exclusion of untreated brain metastases and sponsorship by Roche and Genentech; regulatory and guideline adoption still require further steps.
